Loneliness and the Brain's Connection System
What I see in my clinic is this: Loneliness is often not just a "lack of a social circle" in isolation; it is also about how a person's brain processes signals of trust, intimacy, and contact within a relationship. Some people want people, but intimacy makes them tense. Some want to talk but fear being misunderstood. Some want to be loved but withdraw the moment they need it. So loneliness is sometimes not about having no one outside, but about the brain's inability to read relationships as a safe space.
The Working System of a Brain Struggling in a Crowd
Therefore, when working with loneliness, simply saying "socialize more" is not enough. It is necessary to look at how the person's brain reads intimacy, whether it perceives social contact as a threat or a safety signal, and how it reacts to the possibility of rejection. At this point, I think it is very valuable to use several methods together.
When working with loneliness, the goal is not to throw the person into a crowd. The goal is for the brain to become capable of tolerating connection and intimacy again. Because sometimes a person doesn't need many people, but a safe relationship. And sometimes, they first try to rebuild the feeling of "I am worthy of a relationship" within themselves.
Secondly, the brain's regulation system, which works together with the body, is very important. Because loneliness is not only experienced in the mind; it is also experienced in the body as tightness in the chest, emptiness in the stomach, a lump in the throat, insomnia, restlessness, numbness, or a constant need to check the phone. Breathing, body awareness, sensory focusing, sensory regulation, and safe contact exercises can soften the brain's perception of social threat.
Thirdly, QEEG and neurofeedback like brain-based methods can provide significant support for some individuals. Especially when loneliness is accompanied by anxiety, hyperarousal, obsessive thinking, sleep disturbance, distractibility, depressive withdrawal, or a prominent perception of social threat, it can be beneficial to see the brain's regulation patterns and work with personalized neurofeedback protocols. Neurofeedback here is different from telling the person to "socialize"; it aims to help the brain learn a calmer, more balanced, and more relationship-open working pattern.
Fourthly, relationship skills are worked on. Because loneliness sometimes stems not from not wanting a relationship, but from not knowing how to initiate, maintain, set boundaries, express needs, or repair hurt feelings. The person can learn to reconnect with small and safe steps: sending a short text message to someone, planning a regular weekly meeting, joining a group, asking for help, or being able to say "I would like to come too."

Loneliness and Addiction: What Does the Brain Fill the Emptiness With?
There is often a stronger connection between loneliness and addiction than meets the eye. When a person is left alone, they don't just experience a social deficit; the brain's connection, reward, relaxation, and self-regulation systems are also affected. Therefore, prolonged loneliness can pave the way for cycles like substance, alcohol, gambling, shopping, eating, phone, social media, or relationship addiction in some individuals.
Viewing addiction solely as a "pursuit of pleasure" is incomplete. Many addictive behaviors are actually the brain's attempt to soothe pain, emptiness, restlessness, feelings of rejection, or internal tension. When a person feels lonely, a kind of deficiency signal is generated in the brain. This deficiency sometimes manifests as a need for "someone to see me," sometimes as a feeling of "let something happen so the emptiness inside me stops," and sometimes as an urge like "I can't stand it, I need to relax right away."
This is where addictive behavior comes into play.
Gambling provides short-term excitement. Alcohol creates a feeling of relaxation. Social media provides an illusion of visibility and contact. Eating, nicotine, or shopping provides brief relief to the body and brain. But this relief is not permanent. After a while, the brain feels the same emptiness again and demands the same behavior again. Thus, loneliness and addiction can turn into a vicious cycle where loneliness feeds addiction, and addiction feeds loneliness.
As the person turns to addictive behavior, they often distance themselves further from relationships. Feelings of shame, guilt, hiding, lying, going into debt, loss of control, or being angry at oneself increase. These emotions cause the person to withdraw even more. Loneliness deepens, the brain seeks relief again, and the addictive behavior repeats. So the problem is often not that the person doesn't know what is right; it is that the brain's working system automatically reverts to its old relaxation pattern.
For this reason, when working with loneliness and addiction, simply saying "socialize," "use your willpower," or "don't do this" is not enough. How does the person's brain read loneliness? With what behavior is it trying to close the feeling of emptiness? What automatic pattern does the brain return to when the urge hits? How do the relaxation, reward, and control systems work? It is necessary to look at these.
At this point in the clinic, I try to make visible, together with the person, how loneliness feeds addiction. Questions like "When do you play?", "After what emotion do you drink?", "When do you feel the most empty?", "What is your brain looking for before that behavior?" allow us to understand addiction not as a moral weakness, but as a mislearned regulation pathway of the brain.
QEEG and neurofeedback can open an important space here. Because in some individuals, brain working patterns such as hyperarousal, impulsivity, reward-seeking, attention-regulation difficulties, obsessive repetition cycles, sleep disturbances, or depressive slowing may underlie the addiction cycle. When these patterns are seen with QEEG, neurofeedback can help the person's brain learn healthier regulation patterns.
The important aspect of neurofeedback is that it does not just tell the person from the outside what they should do. The brain learns its own activity through feedback. As healthy regulation patterns are reinforced, the person can learn to pause, wait, and produce a healthier response when the urge hits, instead of reverting to the old automatic behavior. This is particularly valuable in addiction cycles triggered by loneliness.
Because relapse in addiction is rarely caused by a lack of knowledge. The person already knows they shouldn't gamble, drink, put down the phone, or harm themselves. But when loneliness, emptiness, or tension arises, the brain runs the old pattern. With neurofeedback, the goal is for the brain to learn a new regulation pathway instead of repeatedly using this old automatic route.
Understanding the link between loneliness and addiction shifts the focus away from blaming the person.
Being able to say "Your brain has learned to soothe loneliness with this behavior" instead of "You have no willpower" changes the doorway to treatment. Because then, the goal is not just to stop the behavior, but to reorganize the brain's systems of emptiness, contact, reward, and relaxation.
The opposite of loneliness is not just a crowd. The solution to addiction is not just prohibition either. The main goal is for the brain to relearn healthy pathways of connection, trust, satisfaction, and self-regulation. When a person becomes more balanced from the inside, they can approach outside relationships with less fear, less emptiness, and less dependent need.
